Caffeine should help. Have a big coffee or soda and see if that straightens you out.

If you want to turbocharge it then you could take an over-the-counter decongestant like Sudafed. Just take one pill and give it 20-30 minutes to take effect. Make sure to drink lots of water while your on it as it dehydrates you pretty significantly.

You may want to see if you can get a prescription for Ritalin. I don't like it anywhere near as much as Adderall XR but it's dirt cheap in generic form.

If you are used to adderall, then OTC things like caffeine might not work for you very well. One stack that has worked for me was large doses of ALCAR (10g) stacked with a large dose of piracetam (10g) and caffeine (and choline) -- which kept me going for several hours.

However, if you have ADHD, then I would just go to the doctor and get some adderall. Another alternative is to get some modafinil (Provigil). It is a stimulant but seems to selectively work in the brain -- It doesn't stimulate your entire CNS the way adderall does. Another benefit is that the elimination half-life in the body is about 12-15 hours, and so one dose lasts all day (unlike adderall). I've used it, and I gotta say ... The stuff is incredible.

Make sure to test out whatever you're going to use before you use it. Simulating the exam / exam conditions at the same time would be a good idea as well.
