Here's my Max-OT spreadsheet containing the full 6-month roadmap outlined in the Max-OT PDF available on John's training page. The idea was to fit as much info as possible into the smallest space so I wouldn't have to carry a big binder or a clipboard or whatever with me to the gym. I use the white space above the wt/reps columns to record the date, and the italicized bits in the Notes column are the target rep ranges.

It's probably better suited for printing out and writing on than filling out electronically, as the formatting would probably make it hell to get any useful graphs or anything out of it. Personally, I just print it out on a single piece of paper (front and back) then fold it up and carry it around in my pocket at the gym (pausing occasionally to fill in the data, of course :) )
